Lisa Burden leads the delivery of patient care services for Home and Community Care Support Services across the province. This portfolio is fundamental to our mission of assisting our patients to be healthier at home through connected, accessible, patient-centred care. Lisa ensures operations promote effective, high quality patient care that meets the needs of the communities we serve and achieves the best possible health outcomes. This includes developing, implementing, monitoring and evaluating province-wide home and community care performance and accountability activities. Moving towards a modernized home care system, Lisa works collaboratively with Ontario Health, the Ministry of Health, the Ministry of Long-Term Care and Ontario Health Team (OHT) partners to create a roadmap to integrate home and community care services within the OHTs. Under her leadership, the Patient Services team members spearhead and help implement new models of care that can be leveraged across the province.
An innovative and strategic leader, Lisa brings more than 25 years of senior health care executive experience across all sectors of the health care system: community, hospital, long-term care, and Ministry of Health. Over the past seven years, in her role as Vice President, Home and Community Care with Home and Community Care Support Services Central East, she continues to provide strategic leadership, direction and oversight to the development, implementation and evaluation of home and community care programs and services. In this role at Central East, she was accountable for, and to, over 900 Home and Community Care staff. Lisa has cultivated strong relationships with our community stakeholders and service provider organizations that are invaluable to the seamless care delivery in which Home and Community Care Support Services strives to achieve.
Lisa holds a Bachelor of Science in Nursing and several professional development certifications, including LEAN Health Care – Yellow Belt, Negotiation and Mediation Certification. In addition, she completed the Community Health Leadership Program through the Rotman School of Management.
