Accountability, Strategy & Performance

Home and Community Care Support Services is a Government of Ontario Crown agency with accountability to the Ministry of Health, Ministry of Long-Term Care and Ontario Health. We are guided by provincial legislation, including Bill 175, Connecting People to Home and Community Care Act, 2020, Fixing Long-Term Care Act, 2021 and the More Beds, Better Care Act, 2022 

In accordance with government priorities to strengthen and better integrate Ontario’s health care system, we are working to modernize home and community care services within the Ontario Health Teams model of care, so patients’ health care journeys are seamless and they receive better and faster care that meets their unique needs.
