Wound Care
Home and Community Care Support Services offers evidence-based, best practice wound care in alignment with Ontario Health (formerly Health Quality Ontario) wound care standards and Wounds Canada. Specialized wound care teams including Nurse Champions, Nurses Specialized in Wound, Ostomy and Continence, and dedicated wound care coordinators are available to support patients suffering from acute, chronic and complex wounds.
Wound care specialist teams are knowledgeable nurses with an expertise in providing comprehensive wound management which facilitates timely healing and improved quality of life for patients using gold standard treatment options. They collaborate and establish wound treatment plans with community service organizations, primary care providers and hospitals to provide coordinated wound care treatment plans to meet a patient’s specific needs, either at home or in a community nursing clinic. This includes assessing and diagnosing wound type(s), determining patient needs while considering other factors that affect wound healing, in order to develop a customized care plan to support wound management and best treatment options for healing, and improved quality of life and any appropriate self-management techniques.
In addition to supporting wounds, this team specializes in supporting patients alongside their health care team in new ostomies, revisions or changes in an ostomy that require unique approach to appliance and stoma care.
Whether you require this level of specialized care following a surgery, or to support optimal healing for conditions such as diabetic foot ulcers, pressure ulcers, arterial ulcers, vascular leg ulcers, stoma complications or require negative pressure therapy, our care coordinators will ensure all necessary services and supports are in place while also providing education to patients, their families and caregivers throughout every stage of treatment.
