Ensuring Exceptional Care: Home and Community Care Support Services presents Our 2023-24 Annual Business Plan

Home and Community Care Support Services is pleased to present our Annual Business Plan (2023-24), which guides our work and supports us in achieving our mission of helping everyone to be healthier at home through connected, accessible, patient-centred care. 

The plan was framed by direction from the Ontario government, which set a clear and important mandate for Home and Community Care Support Services to play a pivotal leadership role in supporting the modernization of home and community care across Ontario.

Last fall, Home and Community Care Support Services reached out across the province to partners, staff and the people we serve to help determine where we should focus our collective energies and efforts for the 2023-2024 fiscal year. We engaged with more than 1,600 people, including health system partners, service provider organizations, Indigenous and Francophone health leaders, our staff and our provincial community of advisors who represent the patients, families and caregivers we serve.

Thank you to all who helped in the development of our business plan. Your voices were crucial to ensuring our proposed initiatives and objectives would improve patient outcomes and contribute to our collaborative work as a health care system.
