Coordinated Bed Access
Coordinated Bed Access (CBA) is the single point of access for post-acute rehabilitative and palliative bedded levels of care, including:
- Rehabilitation (stroke and general);
- Low Intensity Rehabilitation;
- Complex Medical Management;
- End-of-life/Pain & Symptom Management Complex Continuing Care; and
- Residential Hospice.
Standard program criteria, application form and discharge planning approach are used by all sites to facilitate flow of patients.
