Making a Referral
Anyone can make a referral to us on your behalf and with your consent – a family doctor, friend, family member, caregiver, neighbour and even you, yourself.
Simply give us a call at 1-833-515-1234.

When you contact us, you may be asked some questions to better understand your needs. Then you will either be connected with a care coordinator, or we will refer you to a community program or service that may be right for your needs.
If your health care provider has already made the referral for you, you can expect a phone call from us once the referral has been processed.
